Subscriptions:
Adults: 26-39 years old $100 month
40+ years old. $150 month
Kids: 5-25 years old $50 each with an adult subscription. $100 without an adult subscription. Billed monthly.
This is unlimited visits, physicals, paperwork, emails, phone calls. If you need access to me you have me. I am not an emergency service, but if it is within the scope of primary care I will care for it.
Insurance:
Copays based off of your insurance. I’m currently in network for BCBS. I’m still working on getting in network with the major insurance networks. I have signed with Aetna, UHC and Cigna. I’m accepting patients with insurance on a limited basis and dependent on if your insurance has chronic care management benefits. Please check with your insurance to see if you qualify.
